Residential Facility for adult men and women whose primary diagnosis is a severe mental health disorder and who have experienced multiple psychiatric hospitalizations, and chronic difficulties living independently in the community directly related to their mental health disorder. Four Oaks provides supervision and supports to residents daily from 9:00am to 9:00pm. Personal care services, and medication monitoring are provided to the residents. Eligible residents will experience a therapeutic environment where skills are taught to help manage their mental and physical health, and medication regimen. Staff will assist residents in learning and maintaining habits of basic community and personal living skills with the goal of transitioning to independent living. All residents are actively participating in mental health treatment. Residential staff and case management coordinate this through referrals to appropriate programs, treatment planning and the provision of symptom management skills. Group therapy is provided in the home. The average length of stay varies according to individual needs and ability to transition to independent living. Residents are encouraged to participate in activities outside of the home taking walks, visiting with family and friends, and attending community social functions.
