Our goal is to touch and serve all residents — those in need of general primary care; those who are managing diseases, fighting addiction, depression, or dealing with family matters; and those who are members of minority communities.Community Health & Wellness Partners offers medical care services to patients of all ages — from birth to death. Additional services include Pharmacy, Chronic Care Management, Integrated Behavioral Health, Medication Assisted Treatment, Wellness and Preventative Care.
